Individual Counseling Application

Counseling Guidelines

Thank you for your willingness to fill out the following application. The goal of biblical counseling is to glorify God by applying the truths of scripture to every aspect of life so that you will be more like Christ and experience the fullness of life that He promises.

We are confident that the Scriptures provide us with “everything we need for life and godliness through our knowledge of Him” (2 Peter 1:3). We also realize that we live in a broken world where people are hurting, have little hope, or may be dealing with the effects of their own sin. You may be in one or all of these categories, but take comfort that God knows every detail of your life and has brought you to this place for a reason, that is to bring Him glory, and to conform you more into the image of Jesus Christ (Rom. 8:29, Phil. 1:6). With this said, know that this is a process of change that takes time, effort, and lots of patience.

Not Professional Advice- Although some of the pastoral or lay counselors of this church may be licensed in other fields, such as medicine or psychology, they do not practice as professional doctors, psychologists or psychiatrists in their role as Community Bible Church Biblical Counselors. In this role, they do not provide the same kind of professional advice and services that they do when they are hired in their professional capacities. Therefore, if you have significant legal, financial, medical or other technical questions, you should seek advice from independent professionals not associated with Community Bible Church. Our pastoral and lay counselors will be happy to cooperate with such advisors and help you to consider their counsel in the light of relevant biblical principles. By signing our “counseling guidelines”, you agree to hold Community Bible Church harmless in any and all matters associated with the biblical advice you have received.

Commitment: Your commitment and willingness to change in specific areas of your life will be instrumental to the counseling process, and if applied, will result in “love, joy, peace, patience, kindness, faithfulness, gentleness, and self-control” (Gal. 5: 22-23). As part of the commitment process, we ask everyone who receives counsel at Community Bible Church to:

  1. 1)  Regularly attend church at CBC on Sunday mornings.

  2. 2)  Attend a community group. We will help facilitate you into a group.

  3. 3)  Complete the homework assignments that will be given.

Confidentiality: We appreciate that you are entrusting us with very intimate details of your life, and we take your privacy very serious. With this, there are some reasons that others may be involved in the counseling process:

  1. 1)  At CBC, we are continually training and discipling others to become “lay” counselors, so we may ask you if you’re willing to allow someone in the meetings to observe.

  2. 2)  We ask that you share with your community group leader that you are counseling with us as a form of accountability and additional care.

  3. 3)  If your counselor is uncertain as to how to address a particular problem, he/she may need to ask for help from another counselor, pastor, or elder. However, names can be kept anonymous.

  4. 4)  When counseling someone who attends another church, and there is an issue that needs to be shared with the pastor or elder of that church, we may contact them.

  5. 5)  When there is an obvious danger, threat, or suspicion of harm, we may need to contact proper authorities to intervene.

  6. 6)  When someone is characterized by unrepentant and blatant sin, and refuses to renounce the sin, the command of Matt. 18:15-17 may be implemented to encourage repentance & reconciliation.

Again, it is our privilege to serve you, and to walk through this season of life with you, “looking to Jesus, the author and perfecter of our faith...” (Heb. 12:2). If the counseling guidelines above are acceptable to you, please sign and date. If you are a minor (below the age of 18), you and a parent or legal guardian must sign.
